Sliding Fee Discount Program

Our sliding-fee program allows us to reduce the cost of the care of you and your family. This program is available for all Prism Health Services including primary care and behavioral health.

Don’t have insurance?  Prism Health has trained Enrollment Assisters to help you understand your options and apply for a health insurance plan that works best for you and your family. This is a free service available to all existing and prospective patients of Prism Health.  Just call, email, or stop by for more information.

Fee Waiver Request Form

Prism Health understands that sometimes there are extreme circumstances that impact a patient's ability to pay. A patient experiencing such circumstances that impact their ability to pay their fees may apply for a waiver of fees by completing Prism Health’s Fee Waiver Request Form

A fee waiver may be granted for a one time need or may be granted for a duration of up to three (3) months.

Fee waiver requests will be evaluated and determined by Prism leadership and may include consultation with the patient’s primary care provider (PCP) or behavioral health (BH) provider, if necessary. If approved, a patient may apply again at the end of their approved fee waiver period.

How do I get started?  

To apply for a fee waiver, download and complete the Fee Waiver Request Form and submit it to Prism Health. Determination will be made no later than ten days from the date the Fee Waiver Request Form is received by Prism Health leadership. Patients will be notified if their fee waiver approval or denial by phone, email or MyChart.
